The following services are available, but not limited to...
- An evaluation of the Senior's functional health status, living situation, potential durable medical equipment needs, and provide a mutually agreed upon plan of care.
- Coordinate referrals for home care, personal attendant care, meals, transportation, and other community resources.
- Provide Senior and family education regarding disease, normal aging change, nutrition, medication management, confusion, skin care, and incontinence management.
- Provide ongoing monitoring and reassessment to make sure the plan of care is appropriate, and make recommendations for changes as needed.
- Provide counseling for Seniors and families to support coping and to provide updated information to help them become empowered health care consumers.
- Coordinate transitions to Assisted Living Facilities and Nursing Homes.
- Provide education and referrals for Medicare, Medicaid, and other insurance coverage.
